VOIP on Android with Google Voice

SIP service provider (Register your SIP account: usually call in is free. ekiga.net)

DID service provider (IPKall is the one still offers free number so far, hopefully more)

VOIP client on Android (CSipSimple, SIPDroid etc)

Since you are likely only to get free call in fomr SIP provider, Google Voice could play a key role for pseudo dial out (free so for US and Canada), as well as call-in forward too. Every time you dial out a number, GV will intercept that call out and make the call out for you, after GV call in and connect your DID number first. There is a free Android app named GVoice Callback does this job on Android sytsem.
